  • If you are diagnosed with glaucoma or diabetes
  • Receiving income support, jobseeker’s allowance, or Working Families Tax Credit.
  • Entitled to an NHS Exemption Certificate, or Disabled Person’s Tax Credit
  • The holder of a current HC2 Certificate
  • Registered as sight impaired (partially sighted) or severely sight impaired (blind)
  • Requiring complex lenses (determined by NHS guidelines)
